Ohio State University Screening Tool
Ohio Brain Injury Screening Tool
Many individuals with a brain injury may not identify as someone with a head injury, nor may they have a formal diagnosis on their file. Sometimes a simple question like “Have you been hit on the head?” as opposed to “do you have a brain injury?” may elicit a discussion revealing the type of information you are looking for.
This Screening Tool by the Ohio Valley Center for Brain Injury Prevention and Rehabilitation can be helpful in outlining questions you can ask your client to determine if they have a history of brain injury and some information on how you can assist and refer them for support. If you suspect they may have a brain injury and are not sure how to proceed with support, contact:
This toolkit is meant for educational purposes only. The information within it should not be used to diagnose or treat brain injury in your clients.
